What to bring to your first appointment

Please bring your Medicare card, any concession, pension or DVA card, a list of your current medicines and the name of your previous doctor so we can request your records. The registration form asks about your cultural background because it helps us offer the right health checks and programs. You can also complete the form at reception before your first appointment; allow ten minutes.
